WHAT TO BRING TO YOUR APPOINTMENT

  1. New patient packet filled
  2. Your medical insurance card
  3. Your driver’s license or photo identification
  4. A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
  5. Pertinent information about your medical and surgical history
  6. Any recent labs, X-rays, MRIs, CTs, or appropriate records you may have

PAYMENT INFORMATION


We do not accept selfpay, AHCCCS and Humana. We accept Visa, Master Card, Discover, American Express and online bill pay through patient portal.

REFERRALS


If your insurance requires you to obtain a referral from your primary care physician as in the case of HMO plans, please make sure the referral is sent to us at least 24 hours prior to your office visit or this may cause unnecessary delays or rescheduled visits. Please contact your primary care physician to assist you with this.

AFTER HOURS


If you have an urgent question for the physician after hours, please call the office at 602-344-9567. If you have a medical emergency please go to the emergency room for immediate care.
